
Chesapeake Health Care is proud to announce that it has been awarded three Community Health Quality Recognition (CHQR) badges from the U.S. Health Resources and Services Administration (HRSA) for outstanding achievements in providing high-quality, patient-centered care.
For 2025, Chesapeake Health Care earned the following distinctions:

Advancing Health Information Technology (HIT) for Quality Badge – Recognizing the organization’s commitment to leveraging technology such as telehealth, electronic health records, and patient engagement tools to improve access and outcomes.

Health Center Quality Leader – Bronze Badge – Awarded to health centers whose overall clinical quality measure performance ranks in the top 30% nationwide.

National Quality Leader in Cancer Screening Badge – Highlighting excellence in meeting or exceeding national benchmarks in cancer screening rates, including breast, cervical, and colorectal cancer screenings.

HRSA’s Community Health Quality Recognition (CHQR) program highlights the critical role of federally qualified health centers across the nation. Collectively, HRSA-funded health centers provide comprehensive primary care to more than 31 million people — including:

1 in 15 adults aged 65 and older
Nationally, health centers have achieved significant quality improvements, including:

A nearly 7% increase in patients with controlled diabetes and hypertension

Hundreds of thousands more patients receiving preventive screenings
