 Kate Zimmerman
Kate Zimmerman
Kate rejoins Peninsula Home Care in the Salisbury office as a physical therapist assistant. She previously worked with the Seaford team from 2007-2012 in the same position.  Kate also has experience working in rehabilitation hospitals (both in and outpatient facilities), nursing homes and home health care. She has an Associate’s degree in Applied Sciences and enjoys the one-on-one interactions during treatment with patients. 
 
MIRANDA WOOD COTA
Miranda Wood
Miranda has been hired by the Salisbury team as a certified occupational therapy assistant to educate patients about techniques to make daily living tasks at home easier.  She comes to PHC from Select Rehabilitation and Health South Rehabilitation Hospital.  Miranda has an Associate’s degree in Applied Science in Occupational Therapy from Delaware Technical Community College.  She is a member of Phi Theta Cappa honor Society and Magna Cum Laude and actively participated in her local church as a Sunday School teacher.  Her favorite part of working in home care is assisting the patients one-on-one.
 
Erin GrayErin Gray
Erin joins Peninsula Home Care as a licensed certified occupational therapy assistant. She received her Occupational The rapy Assistant degree from Delaware Technical Community College with honors Magna Cum Laude. With six years’ experience, Erin will be assisting with treatment plans, creating home exercise programs and educating patients and their families about energy conditioning and safety in the home. She currently resides in Worcester County, MD.
 
Lanilyn Eleanor LibanLanilyn Liban
Lanilyn was recently hired by Peninsula Home Care as a physical therapist with six years’ experience. She previously worked in a hospital setting for five years and one in an outpatient clinic. She received her Bachelor’s Degree of Science in Physiotherapy from De La Salle College. Her favorite part of the jobis seeing her patience get better. She currently resides in Wicomico County, Maryland.
